Organizing Your Surgery Records
Keep a record of your surgery that includes:
- date of surgery
- name, address and phone number of institution where you had surgery
- surgeon's name and telephone number
- medical record number
- copy of discharge summary after surgery
- copies of all pathology reports
- copies of any second opinions
- copies of all imaging reports done before and after surgery
If you have more than one surgery, it's easiest if you keep a separate record for each surgery.
