Organizing Your Surgery Records

Keep a record of your surgery that includes:

  • date of surgery
  • name, address and phone number of institution where you had surgery
  • surgeon's name and telephone number
  • medical record number
  • copy of discharge summary after surgery
  • copies of all pathology reports
  • copies of any second opinions
  • copies of all imaging reports done before and after surgery

If you have more than one surgery, it's easiest if you keep a separate record for each surgery.
