Organizing Your Complementary Medicine Records

Because there are a variety of complementary medicine techniques available, it's easiest if you keep a separate record for each technique that includes:

  • type of complementary medicine
  • name, address, and phone number of practitioner
  • number of sessions
  • date and length of each session
  • symptoms treated
  • side effects, if any
  • copies of any reports or assessments done by the practitioner
